Nursing

Coordinated and Person-Centred Care

Clinical Care, Support, and Coordination for Complex Needs

Our nursing services provide essential clinical care, health monitoring, and coordinated support to help individuals maintain wellbeing and independence in the community.

Nurses play a crucial role in assessing health needs, developing and coordinating care plans, managing medications, providing clinical care, and educating participants and their families. They also support daily living activities, respond to changing health needs, and work closely with multidisciplinary teams to ensure holistic care.

Development and review of care plans

Care coordination with allied health professionals
Support for caregivers and families

Continence Support

Incontinence is a widespread condition to describe involuntary loss of urine from the bladder (Continence Foundation Of Australia, 2020). It prevails in over 4.8 million Australians, making it a very common problem (Friendly Society Private Hospital, 2018).

At My Rehab Team, we understand that continence is a sensitive topic, and our nursing team approaches every individual with compassion, respect, and professionalism. Through thorough assessment, practical strategies, and close collaboration with our allied health team, we help clients manage continence concerns and maintain their dignity, independence, and quality of life.

Continence challenges can affect people of all ages and backgrounds, but are particularly common among older adults and those living with neurological conditions such as Parkinson’s disease, stroke, or dementia. Rather than accepting incontinence as an inevitable part of ageing, our team takes a proactive, person-centred approach. They look beyond symptoms to find sustainable, meaningful solutions.

How Our Nurses Support You:

Thorough continence assessments, including bladder and bowel diaries, medical history reviews, and mobility checks
Personalised continence management plans aligned with your goals and lifestyle
Recommendation and fitting of appropriate continence aids, regularly reviewed as your needs change
Education and training for carers and families to support continence with skill and respect
Collaboration with Physiotherapy, Occupational Therapy, and Dietetics to address contributing factors
Monitoring and prevention of continence-related complications such as skin breakdown, urinary tract infections, and social withdrawal

Urinary Management

For clients requiring Indwelling (IDC) or Suprapubic (SPC) Catheter changes, our nurses provide professional, compassionate catheter care in the comfort of your own home. Catheters need to be changed regularly to reduce complications and maintain comfort and hygiene. While catheter bags can often be managed by carers or support workers, catheter replacement requires a qualified clinician.

Our nurses supports urinary management with:

IDC and SPC catheter changes delivered at home
Gentle, respectful, and patient-focused care at every visit
Coordination with families, carers, support workers, and healthcare providers
Reliable, ongoing catheter management for community clients

Collaborative Care

Our nurse works collaboratively with urologists, general practitioners, aged care providers, NDIS participants, support coordinators, and carers to ensure safe, timely, and coordinated catheter care. Our goal is to help clients avoid unnecessary hospital visits while receiving the support they need at home.

Product Recommendations and Quotes

As a qualified Incontinence Nurse, our team can recommend appropriate catheter care and incontinence products, and provide quotes to assist with funding and supply planning for both NDIS participants and private clients.
