NDIS Social Worker Services

NDIS Planning & Support

  • Assists individuals with disabilities and their families in navigating the NDIS system.
  • Conducts assessments to determine the participant’s support needs to enable them to achieve their goals, and aspirations.

Connecting to Community Resources and Services

  • Assists in coordinating and accessing a range of services, including therapy, housing, employment support, and other mainstream supports.

Capacity Building and Skill Development

  • Supports participants in building their capacity, skills, and independence to achieve their goals.

Advocacy and Decision-Making Support

  • Identifies and addresses disability related issues such as exploitation and discrimination.
  • Provide counselling support to the client.

Social workers can provide these services through face-to-face meetings, home visits, phone calls, or video conferencing.

Social Work in Aged Care Service

Aged care services refer to the range of support and assistance provided to elderly individuals who may require help with daily activities, medical care, or social interaction.

Advocacy &  Decision-making in Their Care

  • Offers guidance and resources for decision-making. ( recent service on providing options for retirement villages)
  • Identifies and addresses issues of elder abuse, neglect, and social isolation.

Providing Emotional Support and Counseling

  • Provides counselling and support to individuals to address emotional, psychological, and social challenges related to aging.

As a neurological and complex disability team, we have a client population that is vulnerable to challenges where a social worker can assist. For example, a young stroke survivor who has difficulty walking and communicating; who is alone and isolated, living in public housing and is being physically abused by an intruder. In another instance, a young person with an acquired brain injury who has lost motivation to engage in therapy or to do anything; who is cancelling therapy appointments consecutively and whose family is struggling to know what to do.
